SHOULD I BRING ANYTHING TO MY FIRST APPOINTMENT?

Yes, please print and fill out the intake form and signature page to bring with you. Please make a copy of your Blue Cross/Blue Shield insurance card to bring with you. If you are unable to print out your forms please let me know and I will print them out for you. Also, it may be helpful to bring a journal or note book to use as a healing journal.
